The systemic hypopigmented skin lesions appeared in a 41-year old man suffering from possibly relapsed lepromatous leprosy. Histopathological examination including DOPA staining diagnosed the hypopigmented macules and patches as vitiligo vugaris. During the treatment with only multidrug therapy without other treatment of vitiligo, the both lesions of leprosy and vitiligo showed beneficial effect. Electron microscopial examination made us ascertain that the repigmentation originated in true melanogenesis due to the possible effect of clofazimine.
