A Study on Bacterial Index according to Multidrug Therapy in Multibacillary Leprosy Patients |

A study on the status of Mycobacterium leprae in the skin and nasal smear according to multidrug therapy (MDT) for 227 multibacillary cases including 127 new cases and 100 relapse cases in the institute was carried out and the following results were obtained. 1. In 227 multibacillary cases, the average bacterial index(BI) before starting MDT was 4.81 and average BI at 6 months, 1, 2, 3, 4 and 5 years after starting MDT were 4.46, 4.26, 3.54, 2.92, 2.34 and 1.74 respectively. So average BI declining rate per year was 0.61. 2. No case became negative in BI until 2 years after starting MDT and 2cases(0.9%) 10cases(4.4%) and 35cases(15.4%) became negative at 3, 4 and Syears after MDT respectively. It was considered that the duration of negative conversion depended on BI status before starting MDT 3. The average BI declining rate per year was 0.61 in 145 male patients and 0.61 in 82 feamle patients. So there was no sexual difference in BI declining. 4. The average BI declining rate per year was 0.60 in 127 new cases 3nd 0.64 in 100 relapse cases. So there was np difference in BI declining between new cases and relapse cases, too. 5. The average BI declining rate per year was 0.56 in 170 LL patients and 0.80 in 57 BL cases. So BI declining was faster in BL than LL. This difference may reflect an influence of cell medicated immunity associated with more having in BL as compared to LL type. |
