Á¦¸ñ A STUDY OF THE BONE CHANGES IN LEPROSY PATIENTS WITH PLANTAR ULCERATIONS

¿ä¾à The author investigated the bone changes of feet in 105 leprosy patients with plantar
ulcerations to indicate the incidence, the extent of such changes and to determine
whether bone changes are inevitable and progressive or resulting deformities can be
prevented or arrested.
The results are summarized as follows:
1. 72 cases were male and 33 cases were female (sex ratio, M : F = 2.2 : 1), and
according to Ridley-Jopling's classiffication, 48 cases (45.7 %) were LL, 31 cases (29.5
%) were BL, 22 cases (21%) were BT, and 5 cases (4.7%) were TT.
2. 90% of the total patients showed bone changes of feet, but there were no
significant differences in the incidence of bone changes in each disease type.
3. In nonspecific bone changes, 8 % of patients showed acute osteitis, osteomyelitis
was observed in 33%, periostitis in 14 %, neurotrophic osteitis in 43%, acute arthritis in
6%, chronic arthritis in 21 %, and talus and navicular neuropathies were observed in 5
% of patients.
4. In degree of bone absorption, absorption of one or more of terminal phalanxes was
observed in 60% of patients, and that of middle phalanges, proximal phalanges,
metatarsal heads, metatarsal shafts, and cuneiforms was 57%, 55%, 43%, 33%, and 5%
in order.
5. 19 cases (18%) showed osteoporosis.
6. There were 18 cases combined with claw toes and 13 cases with drop foot, and
among them all the cases except 2 cases in which drop foot developed recently showed
bone changes in the frontal parts of the feet.
7. All the active or acute bone changes could be arrested or healed by proper
treatment.
