Among 58 lagophthalmos cases visited at the Institute for Leprosy Research, Korean Leprosy Control Association, 29 cases were treated with modified temporalis transfer operation and the other 29 cases were corrected by tarsorrhaphy operation, and the following results were obtained. 1) As compared with the Gillies method in temporalis transfer, the modified method which we have carried out has the advantages of a smaller incision, no visible bulge of muscle belly over the temple, and a direct line of action. For these reasons, it is preferred by doctors working in leprosy control organization after short course of training. 2) In all cases which were corrected by the modified temporalis transfer operation, eye closure action was quite satisfactory after biting exercise and there was no visible bulge of muscle belly over the temple. It was confirmed that the majority of corrected cases are kept their eye closed during sleep. No complications were found after surgery. 3) In all cases which were corrected by tarsorrhaphy, eye closure action was weak and cosmetically it was unsatisfied. This tarsorrhaphy is not recommended for the correction of lagophalmos, however, it is considered that primarily ectropion should be corrected by tarsorrhaphy and secondarily lagophthalmos be corrected by modified temporalis transfer for the patients with lagophthalmos accompanied with ectropion. |
