Lymphocyte subsets in the tissues of 13 fresh patients with various kinds of leprosy were studied using monoclonal antibodies and a modified immunoperoxidase (ABC) technique. There was a higher percentage of Leu 4 positive cells from tuberculoid to lepromatous end of the spectrum. The number of T helper/inducer cells were greater in paucibacillary leprosy than those of multibacillary leprosy, and T suppressor/cytotoxic cells were the reverse. Characteristically T helper/inducer cells in paucibacillary form were predominantly in the mantle surrounding the peripheral nerves and T suppressor/cytotoxic cells scattered in and around the granuloma. In multibacillary form, the T helper/inducer and T suppressor/cytotoxic cells were both distributed among the granuloma. OKT 6 positive epidermal Langerhans cells were identified greater number in tuberculoid and borderline specimens than in lepromatous specimens, by contrast, very few number of anti-S-100 positive cells were demonstrated in both multibacillary and paucibacillary forms. Twenty to thirty percent of B lymphocytes were found in only 2 cases of paucibacillary form and one case of borderline lepromatous leprosy. Small and/or none of NK cells were present in both multibacillary and paucibacillary forms without any differences. The results reported here indicate that this technique adds valuable information about the local immune response and classification of patients with leprosy. |
